Gun violence restraining order forms

Ask for a gun violence restraining order

Fill and file forms to start a gun violence restraining order case. Then, have a copy of the forms delivered (served) to the person who is harassing you.

Attachment	MC-025	Use if you need more space to answer some of the questions when you fill out your Petition.
Civil Case Cover Sheet	CM-010	Required when you file any new civil case.
Declaration OR Attached Declaration	MC-030 OR MC-031	Use to for witness statements or to give more information.
Proof of Personal Service	GV-200	Use to prove that you had a law enforcement officer give the person to be restrained a copy of the request, notice of court hearing, and temporary restraining order (if issued) in person.	Sheriff serves your request
Gun Violence Restraining Order After Hearing or Consent to Gun Violence Restraining Order (CLETS-OGV)	GV-130	If the judge grants the restraining order, they will sign this form.
Proof of Service by Mail	GV-250	Use to prove that you had someone 18 or older (not you) mail the restrained person a copy of the Gun Violence Restraining Order After Hearing (Form GV-130). Make sure you can in fact serve Form GV-130 by mail in your case.	Serve your gun violence restraining order
