Ask to move out of California with a conservatee

If you’re a conservator for someone who can't fully care for themselves (called the conservatee) and want to move out of California, you must get the court’s permission first.

  • Fill out court forms

You’ll need to fill out these forms:

📌 Some courts have extra local forms.
Ask the clerk, check your court’s website, or visit your court’s Self-Help Center to find out what else you need.

  • Make copies

Make at least 3 copies of each completed form:

    • 1 copy for you

    • 1 copy for the conservatee's family

    • 1 copy to serve any other required people

    The court keeps the original.

  • File the forms

Take the completed forms and copies to the probate clerk at the courthouse listed on your forms.

    • Give the clerk the originals and copies

    • The clerk will:

      • File the forms

      • Stamp the copies

      • Give you a court date

      • Keep the original Order (form GC-090) for the judge or give it back to you to bring to the hearing

📌 The judge will decide at the hearing if you can move the conservatee to another state.
